• Create user account
• Pay for service
• Access your user dashboard
• Upload state issued ID into your dashboard
• Obtain a copy of your 5 year MVR and forward it to paulfernandez@arizonaduiservices.servicereviews.net

16 HOUR DUI EDUCATION ONLINE

DUI EDUCATION PROCESS:

•     Create your user account
•     Pay for your service
•     If you completed a screening with an facility upload the Screener’s notes into your dashboard
•     Access your education through your user dashboard
•     Complete education in sequential order

1.     Complete a pre-test to check your knowledge
2.     Module 1 – The Physiological Effects of Alcohol and Drug Use
3.     Module 2 – Understanding Blood Alcohol Content/ Alcohol’s Impact on Operating a Motor Vehicle/ Criminal Penalties for DUI Convictions/ Alternatives to Driving Impaired
4.     Module 3 – The Psychological and Sociological Effects of Alcohol and Drug Use
5.     Module 4 – Self-Assessment of Alcohol and Drug Use
6.     Substance Abuse Treatments/Resources Available in the Community for Substance Abuse Treatment/Self-Help Groups like Alcoholics Anonymous (AA), Narcotics Anonymous (NA) and Celebrate Recovery

•     At the end of each module is a short quiz which will be answered in 3-5 short    sentences
•     Upon completing all five modules there is final exam. You are required to earn 70% on the final exam, but will have multiple attempts to achieve 70%
•     Certificate of completion will be available for download once you pass your final exam
• The MVD will be notified of your completed 16 Hour DUI Education course

**Your DUI education will need to be completed in 8 weeks from the start date of your first class**

DUI SCREENING

ALCOHOL/DRUG SCREENING PROCESS:

• Create your user account with Arizona DUI Services
• Pay for your Screening
• Access your dashboard
• Upload your state issued ID to your dashboard
• Schedule a time and day to complete your Alcohol/Drug Screening
• Complete your Alcohol and Drug Screening
• The MVD will be notified of your completed Alcohol and Drug Screening
• Arizona DUI Services will email you a copy of the Screener’s notes and a copy of the MVD confirmation for your records.

8 HOUR SUBSTANCE ABUSE CLASS

8 Hour Substance Abuse Class process:

***Call the court and confirm they will accept Arizona DUI Services online 8 Hour Substance Abuse Class***

• Create your user account with Arizona DUI Services
• Pay for the 8 Hour Substance Abuse Course
• Access your course through your user dashboard

Complete your 8 Hour Substance Abuse Class in sequential order

1. Take a pre test to check your knowledge
2. Lecture 1- AUD/SUD Self Assessment
3. Lecture 2 – The Physiological Effects of Drugs and Alcohol
4. Lecture 3- The Stages of Substance Abuse and Approaches to the Treatment of Substance Abuse
5. Lecture 4 – The Psychological and Socioeconomical Effects of Drugs and Alcohol
6. Final Exam

Pay for your 20 Hours of treatment

Upload your state issued photo ID into your user dashboard

If you completed your DUI Screening with another facility upload your screenings notes into your user dashboard

Access your treatment through your user dashboard

Complete your 20 hours of treatment in sequential order;

2. Session Two of Treatment – Conflict Resolution
3. Session Three of Treatment – Managing Stress
4. Session Four of Treatment – Cravings and Triggers
5. Session Five of Treatment – Healthy Relationships
6. Session Six of Treatment – Healing anxiety and depression
7. Session Seven of Treatment – Family Counseling
8. Session Eight of Treatment – When a Man Loves a Women
9. Session Nine of Treatment – Follow up to When a Man Loves a Women
10.  Session Ten of Treatment – Recap and resources

Once you have completed your 20 Hours of Treatment you will be emailed a certificate of completion. Additionally, if the MVD or the court you are working with needs confirmation of your completed treatment we will forward them a copy of your certificate of completion.
